I have a Rolex model 1400 lys watch worn to where the stem will fall out.  What are your techniques for building these areas up?  I do have a jeweler friend with a laser welder.

Sounds like you need a new stem and/or setting lever probably the easiest way to go is to order a replacement of whichever (or both) of these which is the problem.

Can you close picture the stem and setting lever by itself to see if either is damaged.
