Stem insertion problem Seiko 6602B

This old Seiko 6602B mvt uses the push type release on the setting lever rather than a detente screw. I'm having trouble getting the stem to lock back in properly. The setup is correct and the Setting lever spring arm the pushes the setting lever back is fine. D5 on the posts and grease on the usual areas. 
I've taken the dial off, the calendar parts off and watched the parts interact. It looks like the yoke isn't holding the sliding pinion and this blocks the ability to push the stem in far enough. I have a second spare and it does the same thing. 

Ideas?

Is the yoke bent or not seated correctly. Some pictures to see the issue would help please

Inserting back the stem on 6602's (and 66's) sometimes is a PITA. You probably know that but just in case, you need to press the setting lever axle to insert it, I think it won't go in if you don't press it as it would in many other movements.

But you still need to play a bit until the stem goes in place. Rotate it in different positions until you get it done, it might take you a little bit of time. Some cases are worse than others, I have several 6602-7040 that makes me really nervous when I have to do it
